If the build number of the store.exe file doesn't match the build
number of the same file in the patch, then there's something wrong. Is
it possible that you reinstalled Exchange AFTER installing MS06-029?
The build included with that patch is 6.5.7650.28 but if the patch
wasn't uninstalled before (or maybe after) reinstalling Exchange, the
update may detect that it's already installed and just skip the
update.

As far as where I checked the permissions: AD in the default users OU

Ummm . . . "Users" isn't an OU, it's just a container. It's not a good
idea to keep "regular" users there because policies can't be applied
to containers, only to OUs.

as well as in Exchange at all levels, after doing the showsecuritypage
reg tweak...are there other places I should be looking?

Applying "Send As" from the ESM isn't going to do anything if you have
MS06-029 installed. Check an individual user's "Security" tab in ADUC
and see if the "Send As" is present for the other user in question.
