I dont think it will matter much to just apply it to all of them or
move your BB users into a sep OU and apply there.

Apply to the OU or Users Container instead. You have to select "User
Objects" to locate the "Send As" permissions checkbox.

After I applied the patch, I tried fixing the permissions (explicityly
granting "send as" to the BES service account). But for some reasons the
changes would not "stick."

If you're running Exchange Server 2003 SP1 it does update Store.exe. The
version of this patch for Exchange Server 2003 does not - tested on multiple
servers.

I'm in a similar situation - running 6.5.... I applied the patch and
immediately had problems with blackberry users who were not able to send
messages via their handhelds. Once I removed the patch, back to normal.

Do I understand that you have to be at a certain version level before
applying the patch for it to work properly?
