You are correct, this is a file problem. ESEUTIL /D and online maintenance
only takes care of fragementation problem within the pages of the database.
If the file itself is fragmented, you will need to do something else. You
are on the right track. If I were you, I would make a good backup (online
backup) first, then stop the store service, COPY all of the files off of the
disk on which your information store is located (or at least most of them),
then delete as many of the files as you can, empty the recycle bin, then
copy back on to that disk your mailbox store files. That should reduce the
fragmentation. If you left anything on the disk, you can then run the W2K
defragementer, but cleaning off the disk and then copying everything back on
should give you the most dramatic results.

In larger environments, the backup files are directed to a different disk
partition than the database files. That helps reduce the fragementation
incurred from creating, deleting, and overwriting the backup files every
night. (I'm assuming you are doing "backup to disk".
