Do you see any errors in the Application Event Log?

I came across this type of problem also in these cases:
1. If you have any missing proxy address generator dlls.
2. If you are out of license on your Exchange server.

But if any of these apply to your case you should see some errors in the
Event Log.

Another idea is to run the Exchange Best Practices Analyzer. This is
sometimes helpful to point you in the right direction. For more details
