"Established" doesn't mean a whole lot. SMTP isn't part of the IP
protocol stack. What you want to see is a successful completion of the
SMTP conversation/transaction.

Consider that the problem may not be your server at all! If you see a
successful conclusion to the conversation/transaction in your SMTP
logs then it's time to ask the other end of the connection about what
they see in their logs. And don't forget that the world is full of
broken stuff.

Ummm . . . and what was the TTL on that/those DNS resource records?
That data might be cached on untold numbers of DNS servers all over
the Internet -- and if you count the broken stuff that never refreshes
a cached lookup (like the Exchange 5.5 IMS) you might be spinning your
wheels looking for problems.
Before you remove a DNS resource record, shorten its TTL and then wait
until the original TTL expires (plus the new TTL) before removing the
DNS RR.

It could be anything. Keep in mind that you're dealing with the
Internet and not your LAN. There's plenty of stuff that doesn't work
right and it's not your job to fix it all. TCP/IP and SMTP are robust
protocols. If a connection fails a retry is made--several of them in
fact--at multiple places within the IP stack and the SMTP protocol. If
you have continuous problems for all IP addresses and networks then
it's probably your fault or the ISP's fault. If only a few places fail
then it's not usually worth the effort to find out why unless someone
complains.
Not to sound cavalier, but e-mail is only one means of communication.
The telephone still works. There's IM and, if you're really in a bind,
there's snail-mail and courier services. If you don't get your daily
ration of spam from an open SMTP relay server in Korea or China, does
it really matter?
