MAD.EXE is the System Attendant, or SA service.
I am going to assume:
... that you are running Exchange 2003 with Active Directory 2003.
These steps below are generalized and work with either.
... that you have played the reboot game several times only to experience
this problem again.
... that you can ping the DCs from the Exchange server.
... you are running your hardware on average or above-average servers.
... these servers (2 DCs and 1 Exchange server or server cluster) were
supporting the the collegiate body and administrators properly up until 3
weeks ago.
To start, what I would suggest is this:
Think of any changes in the past 3 weeks (Network topology, DNS, software
installs, etc). Could those have affected AD or Exchange?
Check your anti-virus software and ensure that you are not scanning (or that
you are excluding) the directory/subdirectories housing the SYSVol, staging
areas, ntds.dit, logs, ntds.pat, etc. A good article touching on this is:

If you already have not installed the Windows Server support tools, install
those on the Exchange server and domain controllers.

(Exchange Server)
dcdiag.exe /s:dc-01.sapc.edu
dcdiag.exe /s:dc-02.sapc.edu
netdiag.exe
(Domain Controllers locally)
dcdiag.exe
netdiag.exe
If there are any glaring problems those two applications should catch them.
Are there any events in the event logs on the AD DCs which may be indicative
of a problem?
I know these are a lot of questions, but they will get you started on the
right track and will help in troubleshooting.
