Hi Aaron,

One more thing , have you selected "automatically approve." the check box
under Miscellaneous Custom Form settings" the article you have mentioned is
the bestt soln to the problem. Ensure that you have published the Form under
all public folders and you have installed the form on the profile from where
you are doing mail merge.
