Cannot Login to OWA

When I attempt to login to my Exchange 5.5 OWA account I am prompted to enter
my password 3 times and then after the 3rd time is goes to a blank page.

I am running Exchange 5.5 with service pack 4 and the May 2004 Update Rollup
on an NT 4.0 SP6 server. My IIS virtual exchange directory security
directory properties are set to allow anonymous access and basic
authentication.

I have tried the following;

1. Entering the domain name before the username, domainname\username. I get
the same result as before.
2. Logging into my mailbox with the administrator username and password and
I still get the same result.
3. When I login with the an incorrect password, after it prompts me 3 times,
I get the 401 error.

What can I do?
