Hi ZD,
Some things to check out:
Can you hit the logoff.asp file directly thru a web browser?
(http://servername/path/logoff.asp)
Is the file Read Only?
What are the permissions on the file? Are they correct?
Have you reapplyed E2k Sp3?
Try applying the New April Rollup (https://www.microsoft.com/exchange)
