I am using Windows Vista Home Premium, and have managed to negotiate the fairly complex path of setting up a web site to run ASP.NET with a host header. It's a lot harder than it used to be!
Anyway, I got a site working, and tried to set up a second. Both sites seem to be identical in terms of authorisation, user, etc, but one works fine, and the other gives an error of...
Server Error in '/' Application.
Access is denied.
Description: An error occurred while accessing the resources required to serve this request. You might not have permission to view the requested resources.
Error message 401.3: You do not have permission to view this directory or page using the credentials you supplied (access denied due to Access Control Lists). Ask the Web server's administrator to give you access to 'C:\Users\AY Silver\Documents\TestWebSite2\default.aspx'.

Now, if I look at the problematic site in the IIS management, I can click the "Basic settings" link, and the "Connect as" button shows me that I am connecting as myself (ie the logged on user, not the application user). I can check the permissions on the folder in question, and I have full control.
If I click the "Test settings" button, it gives me two success messages, one for authentication, and one for authorisation. All seems well...
However, if I try to load the site in a browser, I get the above message.
No, your account doesn't have full control on the folder it seems, which can be superseded by another account on the folder, like the Everyone group account doesn't have full control as an example that has superseded your user account rights.
On Vista there is an account on the folder called username (machine-name\username) that doesn't have full rights to the folder, unless you configure it to have full rights, which can even supersede an Admin user's rights on the folder if the two accounts do not match in their permissions.
