To diagnose this will require someone who understands TAPI to get
involved.
Can you test the scenarios without your software or the middleware by
using the MS TAPI Browser directly against the Nortel switch? This
way you could compare the traces from when things work and when they
don't to determine where the problem lies. Specifically you can see
if Nortel's TAPI driver (TSP) is generating connected call states for
the scenarios you are concerned with.
