Low level code to handle events?

Can some kind expert please tell me the basic steps required to handle
events in an Automation client?
Yes, I've read the documentation: too many trees, too many forests! Lots of
detail,but very hard to get the big picture
The app is a mountain of C/C++, so ATL classes may not make a good fit.
