Hi Simon,
From my research, there is no need to use CreateUrlCacheEntry andCommitUrlCacheEntry if you used URLDownloadToCacheFile.
Generally CreateUrlCacheEntry and CommitUrlCacheEntry are used together
with IInternetProtocol::Read or ifstream. You may refer to:

You may check if this issue is caused by your pluggable protocol handler. I
found article regarding pluggable protocol handler. You may check if it is
helpful to you:
