Hi Charles,
This issue has been reproduced on two separate installations, so yes, we have seen it on different machines.
As an aside, the "Fast Mode" was with *Windows Explorer*, not IEXPLORE.EXE.
As a second aside, you mention optimisation of the server web application. *There really is no server side application*. The issue occurs with only a straightforward access to the directory structure on the server.
The issue seems to be peculiar to requests from within Internet Explorer (i.e. we have not witnessed the issue using Windows Explorer directly), however, we do not know whether/how Internet Explorer uses Windows Explorer functionality for the "file-browse" dialog mentioned in my original post. We suspect it may use a system DLL for the file-browse functionality.
To reproduce the behavior, you need a client, a server and a satellite simulator between them. The delay on the link should be set to 500ms to see the problem clearly.
A directory on the server should be mapped to a drive on the client, and a web page should be constructed containing an HTML form with a file-input field.
Clicking the browse button of the file-input field will open the file-browse dialog. When navigating the mapped drive, performance will be slow (as is to be expected), however, when you select a file and click the "Open" button, the dialog can sometimes take as long as 4 minutes to return.
Note: this behavior is typically seen when accessing a folder on the network drive *for the first time*.
If you run filemon to view the exchange between client and server, you see a large number of seemingly spurious attempts by IEXPLORE.EXE to access non-existent files, *and it is these requests that account for the bulk of the 4-minute delay*.
Many thanks for all your help thus far. We have not yet resolved this issue and are currently investigating work-arounds.

From my research, I couldn't find any description on "Fast mode" of IEXPLORE.EXE that you mentioned.
I can explain the latency issue, however it's hard for me to reproduce this issue and estimate the real time spending due to various network conditions.
I recommend that you check whether or not this long latency issue occurs on other machines.
If other machines have the same issue, I would like to know if it is possible that the server web application can be optimized in addition to network latency.
