how to create AVI file, include G.726 audio.

The setting of AVISTREAMINFO and WAVEFORMATEX was wrong or the made AVI
file was not able to be reproduced though I wanted to make AVI file by
using the audio data that contained G.726(40,32,24,16kbit/s).
Please advise a set value of AVISTREAMINFO and WAVEFORMATEX.
