I don't really know. Most programs aren't likely to face this problem
in typical usage but that doesn't mean that any thought was put into
preventing the situation. Exchange Server may be an example where it is
particularly hard to avoid the problem. With the huge number of programs
developed by Microsoft, it's likely that you can find examples of almost any
bad practice, even though the overall quality of their code is pretty good.
