I'm not sure why I'm having such a hard time getting this point across.
IF an app uses a "measure the screen" approach as I described earlier, then
the app CAN know the exact pixels per real-world-inch on the current
monitor in the current resolution with the current settings, and the app
can then DISPLAY the elements exactly as they will "measure in the real
world".
Is it a common need? No, probably not. But certainly not unheard of.
