Why is Assert Failed happening?

Hi,
I'm a beginner in DirectShow programming and i've built and encoder and
decoder filter.
i'd like to know what's the reason whenever i play my filter graph
(using the encoder and decoder i built) the graphEdit crash and ASSERT
Failed error pop up.
there indicates 3 objects left active!
and there's another ASSERT Failed:graphedit.exe - Application Error,
saying that
The instruction at "0x011c7717" referenced memory at "0x00000004". The
memory could not be "read".
May i know what is the reason of these error?
pls guide.
