If cam and nvr is using same (MPEG-4) streaming then why we can't have
interoperability ?

One stumbling block is that IP cameras and tuners often require some kind
of log in sequence. I'm surprised at how many of them require proprietary
software in the path.

people marking their products with MPEG-4 really do their own kind of
stuff under the hood. maybe, because standard it self it quite diverse
and complex.
