msado15 crashes

We have an application that connects to multiple MS SQL Server databases (3
so far) and it works fine but once a while it crashes generating following
error in Event Log:
"The application, C:\iServer\iServer.exe, generated an application error The
error occurred on 11/17/2004 @ 12:24:30.652 The exception generated was
c0000005 at address 4DE14A39 (msado15)"

There is no pattern to this some times it happens 2-3 times a day and some
times it runs for 5-6 days without any problems. The application is written
in VB6 running on XP Pro SP2, it was started with ADO 2.6 but it was chnged
to 2.8.
