1. Is it required to have the knowledge of SQL Server Management/Maintenance?
the case studies are sure to include the use of sql server 2000, as this is the recommended rdbms to be used in .net/msf solutions. you need real world expereience of including this dbms in your solution designs.
2. Apart from RDBMS, what other Data Models are included in the exam?
not sure what you mean by this. i don't think there'd be much on hierarchical DBMSs or OODBMSs/ORDBMSs, if that's what you mean. only sql server's used (specifically 2000 for this exam) and as far as i know, sql server only supports the relational model.
3. How much stress is given on MSF? I have some links to sites/blogs on MSF and I have downloaded the MSF Whitepapers from MS site but I dont have any book specially devoted to MSF.
lots! see http://www.codeclinic.com/70-300skills.htm you really need to know msf and should really have a couple of years real world experience using it on real projects. that's what the exam expects of you. i feel this exam is much different from the others (i've just failed it --first exam of 5 in a row that i passed well on). with the others you could (if you chose) just learn the material and take the exam, but this exam really does test your real world experience of *using* all these technologies together in concert to provide real world mission critical sytems.
4. How much in depth should I know about testing? Are there questions based on specific testing tools?
be careful asking people this type of question, as you're effectively asking people that have sat the exam to disclose (part of) the content of one or more questions.
you need to know your msf stuff, which covers testing in depth.
5. What about technologies like BizTalk Server, Exchange Server, Commerce Server etc.?
very good question. i was caught out a bit by this. i came across a questions in a practice test that assumed working knowledge of MS Host Integration Server and a couple of others i forget now. this goes some way to prove that you not only need to know your stuff, but also have the experience of integrating w/ other MS products such as those you mention. so the answer is "yes".
a strong element of the exam is knowing how to best serve a customer's needs. it might be better, for example, for a client to maintain some of their legacy mainframe computers and use MS HIS to interface w/ them whilst only replacing part of their overall suite of systems/apps w/ a new .net solution.
