The SQL Server Books Online is the ultimate authority, but hard to work
through.
Some people like "Microsoft SQL SERVER 2000 Database Design and
Implementation"
from Microsoft Press, "Professional SQL 2000 Programming" by Rob Viera from
Wrox. Finally, try "MCDBA, MCSE, MCSD, MCAD Training Guide (70-229): SQL
Server 2000 Database Design and Implementation" by Thomas Moore, Ed Tittel
from Pearson Education.
