even if the SQL instance binds to the external interface I would expect ISA to filter incoming connections.
I was surprised as I've normally found the wizards to be very effective.

No, actually, only one was originally installed.
I didn't perform the original SBS install, so I don't know if it was run before the second NIC was installed. I definitely ran it after the second NIC was installed, since that's the first time it's been connected to the Internet.Have you run the ICW? (SBS console, 'Connect to the Internet')
