I would only add and I am sure many would agree here but I always ensure
this is UNTICKED:-
Make sure that Allow all computers which successfully authenticate to relay,
regardless of the list above is selected ... you will see this in your
readings.

Finally are the headers showing the same IP address then block this in ISA
