Coming from me, the "just as lost as you, but I've been on your path
already" guy, this is my understanding of it:
Your Domain Name should point to your router's IP address.
Your router should forward traffic destined for port XXX to your server,
which should be listening on that port.
I have a Netopia 9120XL which is by the same company as your Cayman, so I
assume they share SOME functionality. See this post:

Using that thread and Smallbizserver.net I was able to get up and running.
Its not your exact setup, but the theory should apply. MAKE SURE all your
ISA settings are correct, or you won't get anywhere.
