We currently use a layered approach.

For small dollars per month, our ISP uses an App River product to scan
emails at his level.

On the 4.5 Small Business Server I use a product from TrendMicro.

Nice feature that the TrendMicro product has is that I can get virus updates
on an hourly basis.

We thankfully have never been hit with a virus.
