The baic concept here, is for the Query to be set as the Record Source of the report, and each field in the query needs to be represented by a bound control (probably a textbox) on the report.

I am not sure what you are referring to with the "templete". Maybe it would be better to create the basic report using the report wizard. From the Reports tab in the Database Window, click New, then select 'Report Wizard', enter the name of the query in the 'Choose the table... ' box, and take it from there.
