Input Form



I'm trying to create a form where everyday a user will go in and enter values
for a list of various items. However, I want the item description to be
stored in a field... not have each item desciption as it's own field
containing the value.

How do I build a form that shows me the full list of items (about ten) with
input fields next to them? Also, what's the best way to have a user only
enter the date for the data once, rather than for each line?

I eagerly await assistance from the Access Gods....
